RM 25 (only for
individual categories A, B, C, D and E)
Group Promotion:
RM 270 for 10 person in a single registration *(until the closing date of
registration for the run) *Each participants in the group is considered as
participant in individual category.
RM 30 per
participant for Individual category A, B, C, D and E
RM 50 for Buddy
Category (Each participant will only have to pay RM25)
Closing Date: 15th Nov 2015
Runner's Entitlement - T-shirt, goodies bag & certificate
Prizes for top
three winners will be given for each category.
Registration -
1) Fill in complete
and relevant details in the form.
2) Make payment -
Payment can be
done to CIMB account number 8006934679 (Ching Siew Mooi).
3)After payment is
done, kindly send a scanned copy of the transaction to
diabetesrun2015@gmail.com with details of the following :
Name
Phone Number
Category
Group Name (if
applicable)
4) we will send a receipt with a reference number on it via email to you
once the payment is confirmed.
*For the group
promotion, select a representative to make the payment and we will send the
receipt to the email of the representative.
5)The reference number is as a proof for your registration
and payment
T-shirt Collection
We will have 2
days for participants to collect their T-shirts. The details will be posted in
the FB page - --> HERE
Participants are
required to show their receipt with the reference number on it in order to
collect T-shirt.
Representatives
are allowed (representative have to present the respective receipt).
